OUTREACH COORDINATION
Ensuring individuals receive the awareness and advocacy they deserve.
True Experience’s Community Outreach Program provides vital emotional support, crisis intervention, and assistance with government programs for adults living with mental illness. Our Outreach Coordinator plays a critical role in ensuring individuals receive the guidance, resources, and advocacy they need to live with dignity, independence, and confidence in the community.

Key responsibilities include:

Conducting comprehensive assessments to identify the needs of individuals requiring mental health support
Providing crisis intervention and ensuring emergency services are accessed when necessary
Advocating for clients and raising awareness of mental health issues within the community
Coordinating care with healthcare providers, social workers, community organizations, and government agencies
Assisting with obtaining and renewing important documents, including Ontario Health Cards, photo ID, birth certificates, driver’s licenses, passports, and supporting access to programs such as Ontario Works and Ontario Disability Support Program
Supporting clients in maintaining compliance with Canada Revenue Agency requirements and personal income tax filings

Through these services, the Outreach Program empowers individuals, reduces barriers, and fosters independence, ensuring that each person has the tools and support necessary to participate fully in community life.
