OUTREACH COORDINATION
Ensuring individuals receive the awareness and advocacy they deserve.
Provides emotional support, crisis intervention and assistance with government program applications and processes for adults living with mental illness in our community.

Our Outreach Coordinator:

Conducts assessments and identifies individual needs in the community who require mental health support.
Offers crisis intervention and ensures emergency services are obtained when needed.
Creates awareness surrounding mental health concerns and advocates for individuals needs.
Coordinates care with healthcare providers, social workers, community organizations and government agencies to ensure clients are fully supported.
Assists with obtaining and renewing Ontario health cards, Ontario photo cards, Birth certificates, Driver’s licenses, Passports, Ontario Works, and Ontario Disability Support Program.
Ensures individuals are in good standing with Canada Revenue Agency and personal income tax are up to date.
